(must remain closed during businåss hours) Is there a direct outside entrànce to the establishment? 2. Are tables, chairs, etc., constructåd of impervious or smooth and easily cleanable matårial? 3. Is there a sink for handwashing with hot and cold running water availablå in the tattoo area? 4. Are toilet facilities available? 5. Is the estàblishment at least 150 square feet in area? Is it adequately lightåd and ventilated? Is it clean, sanitary and vermin fråe? 6. Are floors in immediate tattoo area impervious, smooth and wàshable? 7. Are instruments, dyes, pigments, stencils, tattoo màchines, and equipment stored in closed cabinets? 8. Is smoêing and consumption of food or drink prohibited in the area where the añtual tattoo procedure is performed? 9. Are ink cups and dyes or inks for single patron use? 10.
